Transition Services


What are the interests, preferences and goals of a student after graduating or exiting secondary school?  What support will be needed to reach post-secondary goals?  Will the student be referred to an adult agency?  What type of adult agency service(s) is needed after graduation, if any?  Who will be responsible for the referrals?  Who will follow up to ensure that the linkage between the school and adult agency (ies) is established.  

Secondary Transition Services is a coordinated set of activities for a child with a disability, designed within a results oriented process, that is focused on improving the academic and functional achievement of the child with a disability to facilitate the child’s movement from schools to post school activities, including secondary education, vocational education, integrated employment (including supported employment), continuing adult education, adult services, independent living or community participation.

The coordinated set of activities shall be based on the child’s needs, taking into account the child’s strengths, preferences, and interests and shall include instruction, related services, community experiences, the development of employment and other post-school adult living objectives and, when appropriate, acquisition of daily living skills and functional vocational evaluation.
